Medical Health Forms
We are updating our medical records systems to ensure we have the most up to date information on your child's health.
Attached here is the new form that you will need to fill out and return to the school by Friday 30 August. You can drop it off at the front office when you are next at school, or it can be handed directly to your child's teacher.
The new form help us collect your child's important medical information, and the introduction of new systems will mean we'll be able to better manage the daily and emergency health needs of your child.
